There are 3 ways to request the HIV self-test:
1. Complete the online questionnaire (available in English and Spanish).
This questionnaire is required for all individuals prior to receiving an HIV test. If you do not feel comfortable filling out the questionnaire online, see the next option for receiving the HIV self-test.

The initial conversation will take about 10 minutes. The CHIP HIV testing counselor will talk through the process of taking the test and will arrange for a convenient location for us to drop-off the test at a specific date and time.
Once you are ready to take the test, we ask that you call the CHIP team again. This allows our testing counselors to provide the same service they would usually provide in person while testing. The CHIP testing counselor will answer any questions you have while administering the test, ensure you understand the results, and refer you for further services as needed (eg. PrEP, PEP, HIV-care).
We understand that you have many options when it comes to getting an HIV test. As a thank you for choosing CHIP, after you have finished taking the test, the CHIP testing counselor will arrange for a $15 gift card to be sent out to you.
All information provided by you will be kept confidential and secured under our normal HIPAA procedures.
